Intake form

Why intake exists, configuring questions, what clients see

Why intake

After buying coaching, the client fills an intake form — goals, limits, experience, optional photos and measurements before you build assignments. Without it, week one is spent clarifying basics in chat.

Where to configure

On your coaching page in author actions — Configure intake (or similar):

  1. Add questions: text, choice, number, photo.
  2. Mark required fields.
  3. Save before active sales — otherwise early clients see an empty or outdated form.

Changing questions for existing buyers does not rewrite their old answers — plan the form early.

How clients fill it

  1. On the coaching page after purchase — Complete intake.
  2. Answers appear in the client card.
  3. Clients can update answers later on their coaching page (e.g. weight, how they feel).

Example questions

  • 8–12 week goal (fat loss, strength, endurance).
  • Injuries, limits, medications (not “instead of a doctor”).
  • Training and nutrition experience.
  • Home/gym equipment, weekly time.
  • Expected feedback frequency (chat, voice, weekly).
  • Optional photos if relevant to your format.

Do not ask for: passport, full address, card data — not needed for coaching.

After intake

  1. Read answers in the card.
  2. Message in chat within 24–48 hours.
  3. Publish the first assignment block.

Common mistakes

  • 30+ questions — clients abandon halfway.
  • No required injury/limit field — unsafe programming risk.
  • Changing questions every week — hard to compare clients.
